Eligibility: Community Alternatives Program Intellectual/Developmental Disabilities (CAP I/DD) services, or ability to cover private pay costs.
Referral Process: Contact Kathryn Thill or complete an admissions assessment on our referral web page. An initial intake assessment is completed to ensure that we are able to safely and adequately meet the needs of the consumer, inclusive of providing quality staff with proper training, as well as having back-up options to provide additional support.
Billing Arrangements: Community Alternative Program (CAP): directly through Medicaid. Private pay: monthly invoice prior to scheduled services.
