The file below is in Adobe Acrobat format (PDF).  Please download, print, complete, and then sign the application. You can bring your application to the Public Utilities office to pay any applicable fees and schedule activation.  We will also accept credit card payment of your fees over the phone with Visa, Mastercard, or Discover after you fax your application to 919-934-0256.  WE DO NOT OFFER SAME-DAY SERVICE ACTIVATION.

You can have the cost of your monthly water bill deducted automatically from your bank account.  To arrange for this service, please download and complete the form linked below, and fax, mail or bring it to the Public Utilities office with a voided check.
