Community Paramedicine (CP) is a community-based health care program that evolves based on each community's individual needs.
The Johnston County Community Paramedic Program is an innovative way to care for patients outside the hospital.
This Program is comprised of paramedics with 80 hours of specialized training in addition to their paramedic training. This expanded level of training allows the Community Paramedic to collaborate more effectively with all members of the patient's healthcare team in order to support the needs of each patient.
Similar programs in the United States have produced remarkable results.
To qualify for the program, you must:
Live in Johnston County
Be referred into the program by a case manager and have a diagnosis of Congestive Heart Failure, Chronic Obstructive Pulmonary Disease, or Diabetes.
Have utilized EMS 4 or more times in a 30 day period
Have utilized the emergency department 7 or more times in 6 months
Better manage your healthcare at home
Help with medications
Understanding a new diagnosis
Avoid recurring hospital visits
Avoid recurring Emergency Medical Services utilization
Avoid recurring Emergency Department utilizations
Facilitate communication with your healthcare team
Connect you with available resources
